My primary doctor caught my stage 4 CKD and sent me right away to my nephrologists in 2020, but in July 2026, I was sent to a hematologist as my red, white blood count and platelets were low, now taking B12 Singular tablets under the tongue for Iron, so watch that too. Also watch taking any supplements over the counter, I learned that the hard way.

Stage 4 kidney disease can bring on a range of symptoms as the kidneys struggle to filter waste and excess fluid from the blood. Here are the common symptoms to watch for:

- Changes in how often you urinate
- Foamy or bubbly urine
- Fatigue and weakness
- Lower back pain
- Muscle cramps
- Nausea and vomiting
- Poor appetite
- Swelling in the arms, legs, and face
- Itchy skin
- Brain fog (confusion or trouble focusing)
- Shortness of breath It's worth noting that some people may not notice all — or even any — of these symptoms right away, which is why regular check-ins with your nephrologist are so important at this stage.

Keeping a close eye on your kidney function through blood and urine tests helps your care team catch any changes early and adjust your treatment plan as needed.
