An eGFR of 20 places you firmly in Stage 4 CKD (eGFR range: 15–29). This means your kidneys are severely damaged and are only filtering a small fraction of what healthy kidneys would. It's a serious stage, but it's not the end of the road — there are still treatment options available.

At this level, waste products and excess fluid start building up in the body, which is what causes many of the symptoms you may be experiencing. Back pain is a recognized symptom of Stage 4 CKD. Other symptoms that commonly appear at this stage include:

- Fatigue and weakness
- Muscle cramps
- Swelling in the arms, legs, or face
- Nausea and vomiting
- Poor appetite
- Foamy or bubbly urine
- Changes in how often you urinate
- Itchy skin
- Brain fog or trouble concentrating
- Shortness of breath

It's important to let your nephrologist know about your back pain, as they'll want to monitor things closely and rule out any complications.
