Living with a rare kidney disease means having to pay closer attention to what you eat. You may have already started watching your sodium intake. But many people don’t think about potassium or phosphorus until they need to.
Because many healthy foods contain these minerals, knowing what to choose can feel confusing. Here are some tips and recipes to try.
Kidney diseases like C3 glomerulopathy (C3G) and immune complex-mediated membranoproliferative glomerulonephritis (IC-MPGN) can damage the kidneys over time. If kidney function gets worse, potassium or phosphorus levels may become too high or too low.
Also called hyperkalemia, high potassium can become a concern if kidney disease progresses. Very high levels may lead to dangerous changes in heart rhythm. High potassium can also cause muscle weakness, numbness, tingling, or fatigue.
High phosphorus, also called hyperphosphatemia, can cause serious problems over time. Extra phosphorus can pull calcium from the bones, making them weaker.
High phosphorus can also contribute to calcium deposits in blood vessels and the heart, raising the risk of heart attacks and stroke.
Not everyone with a rare kidney disease needs to limit potassium and phosphorus. Your diet depends on your lab results, kidney function, medicines, and doctor’s recommendations.
If you’re not quite sure about your restrictions, ask. Know your limits, and ask a registered dietitian how to apply them in your everyday life.
Some foods are naturally rich in potassium, phosphorus, or both. Others have potassium or phosphorus added during processing.
In addition, oysters, sardines, caviar (fish eggs), and liver are especially high in phosphorus.
These foods aren’t off-limits for everyone with rare kidney disease. One person may need to completely avoid certain foods. Others may just need to watch their portion sizes.
Many vegetables fit into a kidney-friendly meal. Instead of focusing on what you can’t have, choose vegetables that are lower in potassium, if needed.

You can leach (or remove) some of the potassium in vegetables by soaking and boiling them before adding them to meals.
First, peel the vegetable and put it in cold water. Cut it into one-eighth-inch-thick slices and rinse the pieces.
Next, soak the veggie slices for at least two hours in warm, unsalted water. You’ll need to use 10 times the amount of water as vegetables. For example, 1 cup of vegetables requires 10 cups of water.
After a few hours, rinse the vegetable again and put it in a pot. Add five times the water per vegetable serving (such as 5 cups water to 1 cup vegetables). Boil the vegetables and then throw away the cooking water.
Leaching isn’t a perfect science, but it can give you more options. Your renal dietitian can help you understand how this process fits into your meal plan.
Rice and pasta are flexible ingredients that pair well with many lower-potassium vegetables and lean proteins. If cooking with tomatoes, choose canned and drain the liquid.
Using herbs, lemon juice, garlic, onion powder, black pepper, allspice, chili powder, or cumin can add flavor without relying on extra salt. It’s best to avoid too much salt if you have chronic kidney disease (CKD).
If you’ve been using salt substitutes to avoid sodium, you may be getting a dose of added potassium. Check labels for potassium chloride to find out.

Phosphorus is naturally found in many high-protein foods. Lower-phosphorus options include:
Have fresh meats based on your protein needs. Stay away from processed meats, like hot dogs and deli meats, to avoid added phosphates.
Egg whites are naturally much lower in phosphorus than whole eggs. They’re an easy alternative for those looking to limit phosphorus.
Separate the eggs yourself to get the whites. Or you can purchase liquid egg whites in a carton.
The body absorbs less phosphorus from plant foods than from animal foods. Plant-based protein options include tofu, beans, lentils, split peas, and chickpeas. Nuts and seeds can also provide protein, but the right portions may depend on your potassium and phosphorus levels.
Rice milk and almond milk are often lower in phosphorus than cow’s milk, although they usually provide much less protein. When choosing plant-based milks, vegan cheeses, or meat substitutes, check the ingredient list for phosphate additives, including ingredients containing “phos.”
