That's a really important question, and it's completely understandable to want to connect the dots between a new supplement and a change in lab results.

When it comes to melatonin and kidney disease, the honest answer is: it's complicated. Melatonin isn't automatically "safe" just because it's sold over the counter. For people with kidney disease, even supplements can pose risks, including:

- Interfering with other medications
- Containing additives like sodium or potassium that can be hard on the kidneys
- Potentially affecting kidney function directly

A drop in GFR can have many causes, and it may or may not be related to melatonin. But the timing is worth discussing with your nephrologist right away. Before continuing melatonin — or any over-the-counter sleep aid — it's really important to get your doctor's sign-off first. They can also help figure out whether something else might be contributing to the GFR drop.

If sleep is an ongoing struggle, cognitive behavioral therapy (CBT) is actually a doctor-recommended first step for sleep issues in people with kidney disease, and it carries none of the risks that supplements might.
